University Hospitals offers eligible employees two comprehensive medical plans. Both plans cover preventive care at 100% in-network and have an unlimited lifetime maximum benefit.

Both plans are self-funded by University Hospitals, which means that University Hospitals pays the plan’s portion of all medical and pharmacy claims. Health Design Plus will provide administrative services, including member services and claims processing. See who’s eligible for UH medical coverage.
Out-of-network coverage is not offered except for Urgent Care and Emergency Care (outside of the UH Quality Care Network).

- Employees and dependents who live outside the UH Quality Care Network service area will have access to Anthem providers in 2018. You may also access care through the UH Quality Care Network providers.

UH Quality Care Network
The network of providers and facilities available to you will depend on whether you live inside or outside of the Northeastern Ohio counties designated as the UH Quality Care Network service area. The UH Quality Care Network service area includes the counties listed below.
Effective January 1, 2018, Ashland has been added as a county within the UH Quality Care Network service area.
Your 2018 Payroll Contributions
You and UH share the cost of medical/prescription drug coverage. How much you pay depends largely on the medical option you elect and the eligible family members you choose to cover. The 2018 contributions for each medical option are shown here.
In 2018, additional costs will apply for employees who use tobacco and enroll in the UH medical plan – an additional $25 semi-monthly surcharge will be charged. Tobacco cessation programs are available as part of the UH Wellness Program. More details can be found here.
