You are eligible to enroll in UH benefits if your employment is either:
  • UHMG physicians with a FTE status of .20 or higher; or
  • All other employees and UHMP physicians who are regular status employees scheduled to work 16 or more hours a week
  • Medical Plan Only: Employees eligible to enroll in the Medical Plan by meeting ACA work hour requirements

As an eligible employee, you can elect to participate in UH medical, prescription drug, dental, vision, flexible spending accounts and voluntary benefits as applicable when you enroll during Open Enrollment or within 30 days of hire or a qualifying event and after you complete the waiting period specified for each benefit, unless otherwise noted. Your enrollment stays in effect for the calendar year unless you have a change in status.

You may cover eligible family members, including:
  • Your legal spouse, if qualifications are met.
  • Your married or unmarried child(ren) (whether or not they reside with you) up to the end of the month in which the child reaches age 26, including:
    • Natural born and legally adopted children, children placed with you for adoption, step-children, or any other children for whom you or your spouse is named legal guardian, according to a letter of guardianship
    • Biological or legally adopted children for whom the plan is obligated under a Qualified Medical Child Support Order (QMCSO) to provide medical coverage
  • Your unmarried child who became disabled prior to age 26 and is incapable of self-support due to mental retardation or physical or mental handicap and is primarily dependent on you for support and maintenance.

To verify eligible family members and/or disability status, documentation will be required whenever a family member is added to your UH benefits. UH has engaged a third party, the Dependent Verification Center (DVC), to coordinate this process. You will receive a letter sent to your home by the DVC which includes detailed instructions and time frame for submission. It’s important that you respond to the DVC within the stated time to continue coverage for your family member.
