Designate Your Beneficiary for the Pension Plan

The UH pension plan includes a Pre-Retirement Death Benefit.  Employees who are NOT married have the option to designate a beneficiary who will receive a lump sum benefit should the employee die prior to retirement.  If you are married, your spouse is automatically your beneficiary and completing the form below is not necessary.

Please read the directions below to designate your beneficiary:
1. Click here to access the paper beneficiary form
2. Complete the form and return it to the address listed

Pension benefits revert back into the plan for unmarried employees who do not have a beneficiary listed with UH Pension Services.
